AI in Care Transitions: Randomized Trial Shows LLM Chatbot Improves Specialist Workflow and Information Quality
A randomized controlled trial published in Nature Medicine evaluated a large language model-based chatbot designed to improve primary to specialist care transitions, one of the most persistent inefficiencies in outpatient medicine. Co-designed with clinicians and patients, the tool was tested in a pragmatic real-world setting involving more than 2,000 patients, 24 specialties, and over 100 specialists across two medical centers. Before appointments, the chatbot conducted structured history taking, elicited symptom timelines and patient priorities, summarized relevant medical history, generated preliminary diagnostic considerations, and produced standardized referral reports for physician review. Participants were randomized to autonomous AI use, AI with staff assistance, or usual care, and compared with usual care. AI-assisted visits were associated with approximately a 29 percent reduction in specialist consultation time. Physicians reported higher quality referral documentation, improved clarity of pre-visit clinical information, and better coordination between primary and specialty care, while patients reported feeling more prepared and better understood during consultations. Safety monitoring did not identify increased clinical risk, and outcomes were consistent between fully autonomous and assisted arms, supporting feasibility for scalable deployment. The study also highlighted reduced redundant questioning and more efficient use of visit time, allowing specialists to focus more on clinical reasoning and decision-making rather than information gathering.
TAKEAWAY: This trial represents a meaningful shift from retrospective algorithm validation to prospective clinical evaluation of workflow impact. It demonstrates that LLM-based systems can function as structured pre-visit intelligence tools that enhance documentation completeness, reduce cognitive burden, and increase specialist throughput without compromising oversight. As health systems face rising referral volumes and workforce constraints, AI-supported intake models may offer a scalable strategy to improve access, care coordination, and operational efficiency while preserving clinician judgment.
